This Shared Insights forum took place on 18 November 2025.
This session focused on the practical implications of an increasing trend towards corporate and gross negligence manslaughter investigations including:
- The legal framework.
- Relationship with independent and public inquiries.
- Interactions with internal processes including disciplinary and other internal investigations.
- Implications for inquest preparation.
- The status of reflective notes.
We also covered the legal framework and looked to previous prosecutions and ongoing investigations for lessons about what differentiates those circumstances from other patient deaths.
